Provider name(s) and credentials
Enter the provider’s name and credentials exactly as they should appear on the claim form.
Select provider specialty(ies)
Check each applicable box to the left of the listed specialties that apply to the provider’s practice. |

Add Legacy ID(s), Tax ID(s), and NPI(s) |
| |
| |
|
Most insurers and payers require some form of Legacy ID (unique provider identification). You must first contact the payer directly to acquire their specific Legacy ID, which is required for all electronic and paper claim submissions. Certain payers require different IDs for electronic and paper claims, so you may have two IDs for one payer.
You may use you Employer Identification Number (EIN) or your Social Security Number (SSN) as your Tax ID. Most providers are now obtaining an EIN instead of using their SSN for their practice to increase their personal security.
As of May 23, 2007, all providers must have and use a National Provider Identifier (NPI). To obtain your NPI, click here. |

Billing Entity(ies) |

There are two ways to add your Billing Entity(ies). You can click on Practice & Payers tab to access the My Billing Entities sub tab and either click the plus icon or click the New Billing Entity Wizard .
Adding a Billing Entity(ies)
To follow the New Billing Entity Wizard , simply follow the instructions listed on each screen. If all the required data is not entered, an error message will appear indicating which field(s) need data before you can proceed to the next screen. Enter the appropriate information. Click Save.
If you choose to click the plus icon , the data fields for the billing entity appear on screen. Enter all applicable information and click Save. You can add more billing entities if the need arises.
Since ZipClaims auto-populates claim data fields using billing entity information supplied for the selected provider, you must choose at least one billing entity before entering charges for a patient and creating a claim. |

There are two ways to add your practice information. You can click on Practice & Payers tab to access the My Addresses sub tab and either click the plus icon or click the New Address Wizard .
Adding a Practice Address(es)
To follow the New Address Wizard , simply follow the instructions listed on each screen. If all the required data is not entered, an error message will appear indicating which field(s) need data before you can proceed to the next screen. Indicate whether the address is a service and/or billing address. Enter the appropriate information. Click Save.
If you choose to click the plus icon , the data fields for the address appear on screen. Enter all applicable information and click Save. You can add more addresses if the provider practices at more than one location or if the billing address differs from the service address.
Since ZipClaims auto-populates claim data fields using address information supplied for the selected provider, you must choose at least one service address before entering charges for a patient and at least one billing address before creating a claim. |

Adding a Payer(s)
To follow the New Payer Wizard , simply follow the instructions listed on each screen. If all the required data is not entered, an error message will appear indicating which field(s) need data before you can proceed to the next screen. Enter the appropriate information. Click Save.
If you choose to click the plus icon , the data fields for the payer appear on screen. Enter all applicable information and click Save. You can add more payers if you choose to send claims to another insurance company.
Since ZipClaims auto-populates claim data fields using payer information supplied for the selected provider, you must choose at least one payer and one secondary payer, if applicable, before entering charges for a patient and creating a claim. |
