One-Time Setup and Training Fee - $299.00

Our Ultimate Customer Care team assists you and your billing staff in setting up your practice information, inputting insurance routing numbers, and in creating your first claims. Our Ultimate Customer Care team is available by e-mail at Support@ZipClaims.com or by phone at 1-866-925-2317, Monday through Friday from 8:00 a.m. through 5:00 p.m. Mountain Time.
 
Monthly Charges

Per-Claim Subscription – Best value for practices generating less than 67 claims per month. Allows for one or many practitioners.
  $15.00 per month per practitioner minimum charge (includes 10 free claims)
 
  $1.50 per claim after the first 10 claims each month
 
Unlimited Claims – Best value for practices filing more than 67 claims per month.
  $100.00 per month for 1 practitioner*
 
  $190.00 per month for 2 practitioner*
 
  $270.00 per month for 3 practitioner*
 
  $320.00 per month for 4 practitioner*
 
  $400.00 per month for 5 practitioner*

Monthly charges do not occur until a full month has passed following the month when the account is created. Thereafter, billing occurs on the first of each month.

*Charges are billed for each practitioner who supervises care and whose name appears in field 31. of a 1500 standard health insurance claim form.*

  c. Entering a Promotion Code
 
    If you have a promotion code, enter it into the Promotion Code box and click Apply. If you do not click Apply, the promotion code will not be applied to your purchase. The Promotion Code authorizes the user to one free electronic copy of the most current edition of ABC Coding Manual for Integrative Healthcare, a $79.00 value.

  d. Terms of Use and Shopping Cart
 
    After selecting your account type in the Choose Subscription section, the shopping cart will provide your subscription subtotal. Monthly charges do not occur until a full month has passed following the month when the account is created. Thereafter, billing occurs on the first of each month.

Once you have proceeded to the Credit Card portion of the Shopping Cart, complete every field in the data form. You must read and accept the ZipClaims Terms of Use to create an account. Check the credit card data to make sure all information is correct. Incorrect information could result in inaccessibility to your account.

  e. Billing / Contact
 
    Click Next to proceed to the Billing / Contact portion of the Shopping Cart. Fill out all data fields and click Next to proceed to the Login / Setup screen. Enter the specified information.

  f. Summary
 
    The Review / Complete screen shows the final invoice total, along with the account address and contact information. If you are satisfied with your final summary invoice, click Finished to create your account. You will receive an e-mail confirming that your account has been created. This e-mail will describe how our Ultimate Customer Care team will aid you in setting up your practice. If you do not receive an e-mail, call us immediately at 1-877-621-5465. For help setting up your practice, e-mail us at Support@ZipClaims.com.

  g. To Set Up Your Practice ...
 
    Click the link below to access the New Account Set Up form. After filling out the form, please fax it back to our Ultimate Customer Care team at 1-505-875-0002.
